Under general supervision and working as part of the Outreach Team provides Needs Assessments and on-going intensive mental health interventions to persons who have a severe and persistent mental illness and who are homeless. Completes and maintains progress notes, service logs and other pertinent documentation according to agency and regulatory service standards.

Provides an array of community based, mobile services that address the individualized mental health needs of persons who have a severe and persistent mental illness and who are homeless, including but not limited to ongoing assessment of needs, coordination of services, linkage, symptom monitoring, advocacy, outreach, housing placement, and skill development. Services may be provided face-to-face or by telephone.

Provides needs assessment, as appropriate, to identify client needs, recommendation for services and recommendations for other assessments.

Conducts outreach to locate, identify, and initiate the engagement process for individuals eligible for services by frequenting locations throughout CuyahogaCounty (eg, streets, alleys, under bridges, abandoned buildings, parks, transit stations, meal-sites, shelters, drop-in centers, libraries, malls, airports and other areas in the community).

Establishes and maintains a working relationship with clients to ensure transition to the most appropriate service provider by providing a safe, non-threatening and supportive environment and utilizing creative and effective engagement techniques.

Initiates appropriate action and crisis intervention with individuals/clients or situations through prompt utilization of community resources.

Ensures compliance with productivity standards and documentation/recordkeeping for agency, funding and accreditation requirements by maintaining and reporting accurate and timely documentation and statistical data.

Supports the integrity and self-determination of clients by promoting empowerment skills.


Bachelor's degree in social work, psychology, sociology or related field and one year applicable work or internship experience, or a minimum of four years professionally supervised work in the delivery of mental health, chemical dependency and/or social services. Must have a valid, Ohio Drivers License with less than 7 points.

Pay: $27,000 to $34,000 USD
