Responsible for overall care management and care coordination including but not limited to health care linkage, treatment of mental health symptoms, alcohol and other drug abuse issues, with a focus on increasing self-sufficiency and self-determination. The services are designed to be pro-active and incorporate a philosophy of prevention services and focus on strategies to help clients change aspects of their behavior, in regard to physical and mental health and addiction.
Some of the other responsibilities include:
Conduct case reviews with integrated care team. Communicates with psychiatrist on a regular basis to facilitate interdisciplinary collaboration.
Conduct comprehensive assessment in partnership with the integrated team and develop a patient centered integrated care plan.
Assist the client with implementation of the integrated plan.
Perform medication management, including medication reconciliation;
Track tests and referrals and work, and follow-up as necessary
Coordinate, facilitate and collaborate with the consumer, team of health care professionals and other providers, and the consumers' family, guardian and/or significant others;
Share the crisis management and contingency plan, assist with and coordinate prevention, management and stabilization of crises and ensure post-crisis follow-up care is arranged and received.
Qualifications:Must be a Registered Nurse - RN
